Referring Doctor Information
Our goal is to provide your patient with outstanding retina care in a timely, efficient fashion. We have made an effort to include a few items in this website that will hopefully make the process of referring patients to our practice easier.
First, we have developed an online form that, by clicking Refer a Patient, will send us information entered about your patient. This will be checked daily and we will get in touch with your patient as soon as possible. Second, we have included our standard referral pad in PDF format that can be downloaded and filled out. Third, we have included the forms in PDF format that we usually have our patients complete prior to their visit. If these can be downloaded and completed prior to their visit, it will expedite their check in process. If they are unable to do this prior to their visit, it is not a problem as we will simply have them complete these forms when they visit our office.
